Left frontal lobe N-acetylaspartate levels in patients with deficit schizophrenia

A B S T R A C T 

Abstract
Objective: This study aimed to detect the N-acetylaspartate levels of
the left frontal lobe in patients with deficit schizophrenia.



Method: Schizophrenia was diagnosed using the DSM-IV criteria.
Deficit syndrome was screened by means of the Schedule for the
Deficit Syndrome. The subjects were 27 patients with schizophrenic
categorized as deficit (N=13) or nondeficit (N=14) and 10 healthy
subjects. Severity of illness was measured using the 18-item Brief
Psychiatric Rating Scale. Levels of positive and negative symptoms
were measured using the Scale for the Assessment of Positive
Symptoms and the Scale for the Assessment of Negative Symptoms.
Proton magnetic resonance spectroscopy was performed to measure
the levels of metabolite (N-acetylaspartate, creatine, and choline) in
left frontal lobe.



Results: Level of N-acetylaspartate (NAA) of patients with the deficit
syndrome was negatively correlated to the Scale for the Assessment
of Negative Symptoms total scores. The patients with the deficit
syndrome had significantly lower level of NAA than did the healthy
subjects or nondeficit patients. In addition, the patients with the deficit
syndrome had significantly lower ratios of choline than did the healthy
subjects.



Conclusion: As N-Acetylaspartate levels could reflect neuronal
density, this finding suggests a neuronal loss in the left frontal lobe
(hypofrontality hypothesis) of patients with deficit schizophrenia.

Defisit şizofreni olgularında sol frontal lob N-asetilaspartat düzeyleri

Özet
Amaç: Bu çalışmada, defisit şizofreni olgularında sol frontal lob Nasetilaspartat
(NAA) düzeylerinin saptanması amaçlanmıştır.



Yöntem: Araştırmaya DSM-IV ölçütlerine göre şizofreni tanısını karşılayan 27 şizofreni (13 defisit, 14 defisit-olmayan) ve 10 sağlıklı kontrol
olgusu alınmıştır. Defisit ve defisit-olmayan şizofreni ayırımı Defisit
Sendromu Çizelgesine göre yapılmıştır. fiizofreninin belirtileri Kısa Psikiyatrik
Derecelendirme Ölçeği, Pozitif Belirtileri Değerlendirme Ölçeği
ve Negatif Belirtileri Değerlendirme Ölçeği ile değerlendirilmiştir. Proton
magnetik rezonans spektroskopi ile grupların N-asetilaspartat
(NAA) ve diğer metabolit (kreatin, kolin, NAA/kreatin, NAA/kolin, kolin/
kreatin) değerleri saptanmıştır.



Bulgular: Sol frontal lob NAA düzeyleri ile Negatif Belirtileri Değerlendirme
Ölçeği puanları arasında negatif bir ilişki olduğu bulunmuştur.
Defisit olguların defisit-olmayan ve kontrol grubuna göre daha düşük
NAA düzeyine sahip olduğu görülmüştür. Ayrıca, defisit grubun kolin
değerlerinin kontrol grubundan düşük olduğu saptanmıştır.



Sonuç: NAA nöronal yoğunluğu yansıtabildiğinden, bu bulgular defisit
şizofreni olgularının sol frontal loblarında nöron kaybı olduğunu (hipofrontalite
varsayımı) desteklemektedir.
